Summer Alliance Meeting - Thursday, July 27, 11am - 1pm
Anthem BCBS Colorado is seeking services of Community Health Worker (CHW), Patient Navigators (PN) and/or Social Workers (SW) for a High Needs High Cost (HNHC) member pilot program in which the CHW/PN/SW program vendor will receive referrals from Anthem BCBS Care Management and/or from the attributed PCP/EPHC group. It is desired that the contracted vendor will provide telephonic out-reach in addition to hospital and home visits for assessment, engagement and follow up. Additional documentation of critical strategies and outcomes in compliance with a standard anthem report format on a Monthly/quarterly basis. The goal of the pilot is to determine if integration of CHW/PN/SW into the care management team will decrease ER utilization, decrease inpatient hospitalizations, decrease readmissions, lower lengths of hospital stays and increase primary care connectivity. Other expectations include but are not limited to collaboration with Anthem’s Care Management team and the member’s primary care physician on findings and care plan development, determination of and removal of social determinants to health. It is also expected the vendor will participate in monthly and quarterly clinical panel discussion with Anthem Care Management and Provider Practice groups participating in the pilot project. Are you interested in learning more about cultural competency and utilizing
cultural competency resources to best fit your organizational needs?
The Southeastern Health Equity Council (SHEC) released its Cultural Competency Resource Guide in 2015. The guide is comprised of resources, trainers, institutions, and publications about cultural and linguistic competency that can be shared with partners to help address cultural barriers with health care systems. Additionally, this guide includes important terms to become familiar with to develop a common language around cultural competency. To follow up on the release of the Cultural Competency Resource Guide, the SHEC will host a series of webinars. The webinars will include speakers from organizations that focus on cultural competency.
The focus of the webinar will be the Community Engagement and Cultural Diversity Program at the Miami Clinical and Translational Science Institute (CTSI) and the services it provides to bring cultural and linguistic assistance to the Hispanic/Latino community in Florida.

Presenting: Andi Dwyer - National Patient Navigation Sustainability Toolkit with American Cancer Society National Colorectal Roundtable
The National Colorectal Cancer Roundtable and the Colorado School of Public Health will be will be hosted a webinar on Jan. 10th to walk users through the new toolkit. NCCRT Steering Committee member Andrea (Andi) Dwyer of the Colorado School of Public Health walked through the toolkit content. The webinar gave an understanding of what’s in the kit, how it works and the real world examples that informed its guiding principles.

Regional Health Connectors Meet-n-Greet

Presenting: Mr. Nick Flattery
Nick Flattery is a senior project manager at CPC Community Health in Aurora, CO. He runs the statewide chronic disease prevention program, Colorado Heart Healthy Solutions. He has over 15 years in the field of community and public health and is passionate about reducing the burden of health disparities throughout Colorado and beyond.
Nick presented on a proprietary decision support and data collection system created for the Colorado Heart Healthy Solutions program, called OSCAR (Outreach, Screening and Referral System). OSCAR allows CHWs in the field and across the state to collect data on program participants and sync that data for analysis to the CPC offices in Aurora.
